Notice of Violation Form. Vendor shall prepare and provide to City a Notice of Violation Form that provides, at a minimum, the following information: a. name and address of the owner of the vehicle involved in the violation; b. the registration number of the vehicle involved in the violation; c. reference to the City’s Ordinance violated; d. the location of the intersection where the violation occurred; e. the date and time of the violation; f. a copy of the recorded image of the violation; g. the amount of fee and charges imposed and the date by which the fee and charges must be paid or appealed; h. instructions on all methods of payment for the fee; i. a clear statement of the time limit to file an appeal and describing the procedure for appealing the violation; x. a statement that the City’s traffic violation officer has reviewed and observed the recorded images evidencing the violation of the Ordinance and has found reasonable and probable grounds to believe that a violation has occurred and can identify the license tag number of the violating vehicle; and,

(d) Vendor acknowledges that upon initial notification from Vendor, the District, as the educational agency with which Vendor contracts, has an obligation under Section 2-d to in turn notify the Chief Privacy Officer in the New York State Education Department (“CPO”). Vendor agrees not to provide this notification to the CPO directly unless requested by the District or otherwise required by law. In the event the CPO contacts Vendor directly or requests more information from Vendor regarding the incident after having been initially informed of the incident by the District, Vendor will promptly inform Xxxxxx Xxx, Data Protection Officer or his/her designee.

  • Notice of Non-Renewal Consultant understands and agrees that there is no representation, implication, or understanding that the City will request that work product provided by Consultant under this agreement be supplemented or continued by Consultant under a new agreement following expiration or termination of this agreement. Consultant waives all rights or claims to notice or hearing respecting any failure by City to continue to request or retain all or any portion of the work product from Consultant following the expiration or termination of this agreement.
