Officer Designated Representative Access to Officer File. An officer may notify the Human Resources department, in writing, that his/her designated representative may be allowed access to the officer's personnel file. This notification shall be recognized for a period of one (1) week unless stipulated otherwise. The officer’s designated representative will be responsible for the protection and security of information provided and will assume any liability which may result from any improper disclosure or use of the information provided.

  • Access to Personnel File Each employee shall have reasonable access to his/her personnel file for the purpose of reviewing any evaluations or formal disciplinary notations contained therein, in the presence of the Director of Personnel or designate. An employee has the right to request copies of any evaluations in this file.
