Orthodontic Coverage. An orthodontics policy is available for the employee and their dependents. Premiums shall be paid by all employees through payroll deductions. Premium payments shall be established through agreement with the orthodontic coverage provider. This policy is subject to premium costs, eligibility requirement, age limitations, coverage exclusions and all other provisions set forth in the applicable insurance contracts.

Orthodontic Coverage. The Company shall provide an orthodontics plan after the employee has been continuously employed for one (1) full year. Benefits will be limited to a maximum lifetime benefit of Three Thousand dollars ($3000) per person. Appliances lost, broken, or stolen will not be replaced.
Orthodontic Coverage. The Employer shall provide orthodontic coverage with the Employer paying one hundred (100) percent of the premiums. Coverage will provide for a lifetime maximum of three thousand (3000) dollars to be paid on a fifty/fifty basis by the Employer and the employee for individuals of age eighteen (18) or less.
Orthodontic Coverage. During the term of this MOU, employees represented by DAIA may be polled to determine if a majority of the membership desires the addition of an orthodontics benefit under the Delta Dental plan. If a majority of members desire an orthodontics benefit be made available, the County and DAIA will meet and confer on the effective date of coverage, plan design, costs, limitations and exclusions. The costs for the added benefit will be borne by each enrolled Delta member whose eligibility for benefits is under DAIA’s program.
