Outcomes and Outputs (Performance Measures. At a minimum, the Sub-Recipient shall: a. Ensure the provision of the services described in this contract are in accordance with the DOEA Programs and Services Handbook, the Sub-Recipient’s current AAAPP- approved Application, and Section II.A.. b. Timely and accurately submit to the AAAPP all documentation and reports described in Attachment I, Section II.E. c. Timely and accurately, per Attachment X, submit to the AAAPP Attachments XI and XII, Exhibits 1 – 2 . d. Develop and document strategies in the Application Update to support the AAAPP’s standard of performance achievement, including increases in the following: i. Percentage of most frail elders who remain at home or in the community instead of going into a nursing home; ii. Percentage of APS referrals who need immediate services to prevent further harm who are served within seventy-two (72) hours; iii. Percentage of active clients eating two or more meals per day; iv. Percentage of new service recipients whose ADL assessment score has been maintained or improved; v. Percentage of new service recipients whose IADL assessment score has been maintained or improved; vi. Percentage of caregivers who, after service intervention, self-report being very confident about their ability to continue to provide care; and vii. Percentage of customers who are at imminent risk of nursing home placement who are served with community-based services.
