Outcomes-Focused Primary Prevention Service Activities. To assist the State in fulfilling federal expectations and mandates, Subgrantee should demonstrate how implemented prevention service outcome-focused work plans incorporate activities that fall under each of the strategies designated by the Center for Substance Abuse Prevention (CSAP) and as indicated by local needs assessment. These outcome service activities shall be developed using the Strategic Prevention Framework (SPF), which DAODAS has adopted as its planning model. Prevention services should follow the model for all services provided and be reflected and documented in the IMPACT reporting database. If Subgrantee is not utilizing all six (6) CSAP strategy areas, Subgrantee is asked to notify DAODAS regarding which strategy will not be utilized in its service area by completing and uploading the CSAP Strategy Checklist into BOX Enterprise by July 31. It is the responsibility of Subgrantee to upload a revised checklist and to notify DAODAS if any plans change throughout the fiscal year. The notification should state which CSAP strategy area(s) are affected; a clear example of how the strategy is being met by another partner/organization; and/or documentation of the lack of need for the strategy to be conducted in the county based on relevant needs assessment data. Subgrantee is asked to work closely with its Regional Coach before submission of the form. The uploaded form will serve as documentation for DAODAS to incorporate in required federal reporting of the utilization of the SABG in South Carolina. The six (6) CSAP strategies are as follows:

  • Preventive Care and Early Detection Services This plan covers, early detection services, preventive care services, and immunizations or vaccinations in accordance with state and federal law, including the Affordable Care Act (ACA), as set forth below and in accordance with the guidelines of the following resources: • services that have an A or B rating in the current recommendations of the U.S. Preventative Services Task Force (USPSTF); • immunizations recommended by the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention; • preventive care and screenings for infants, children, and adolescents as outlined in the comprehensive guidelines supported by the Health Resources and Services Administration (HRSA); or • preventive care and screenings for women as outlined in the comprehensive guidelines as supported by HRSA. Covered early detection services, preventive care services and adult and pediatric immunizations or vaccinations are based on the most currently available guidelines and are subject to change. The amount you pay for preventive services will be different from the amount you pay for diagnostic procedures and non-preventive services. Preventive Office Visits This plan covers the following preventive office visits. • Annual preventive visit - one (1) routine physical examination per plan year per member age 36 months and older; • Pediatric preventive office and clinic visits from birth to 35 months - 11 visits; • Well Woman annual preventive visit - one (1) routine gynecological examination per plan year per female member.

  • Service Animals Humber Residences acknowledges the rights of persons with disabilities to retain their service animal while living in Residence. In order to preserve the health and safety of all people and animals living or working in the Residence environment, the Resident will notify the Residence Office that they require a service animal and will provide documentation as outlined in the Accessibility for Ontarians with Disabilities Act confirming that the Resident requires the service animal. The Resident will also complete a Service Animal Agreement with the Residence Manager or designate, and agrees to adhere to the requirements within it.
