People Eligible for Learning Disability Services. The following refers to eligibility for services and should not be confused with priority of need. People will be eligible for Community Learning Disability Team services if they fulfil all of the following criteria: • Known to have a definite diagnosis of learning disability. There should be reliable recent psychological reports if people have a borderline intellectual ability or fall within the agreed Vulnerable Adults criteria (see below) • Over the age of 18 (but the Community Learning Disability Team will work appropriately with Children and Family Services towards well managed transition to Adult Services, in accordance with the Service Level Agreement between Social Services department divisions) • Presenting social or health care needs identified by assessment which falls within the priority criteria for each service • Ordinarily resident within the London Borough of Barking and Dagenham.
