Adult Services. Adult service performance measures shall be assessed 37 calendar days following the close of Quarter 2 (measuring Quarters 1 and 2) and Quarter 4 (Measuring Quarters 3 and 4). Quarters start September 1st of each year and end August 31st of each year. Remedies and sanctions associated with failure to achieve the following measures are specified in Article VII of the Local Mental Health Authority Special Conditions, as applicable.
1. Adult Service Target: Target is 3,315. This target equals the average monthly number of adults authorized in a FLOC (as defined in Information Item C, which is posted at xxxxx://xxx.xxx.xxxxx.xxx/doing-business-hhs/provider-portals/behavioral-health- services-providers/behavioral-health-provider-resources/community-mental-health- contracts). The statewide performance level for this target is 100%.
2. Adult Monthly Service Provision: An average of at least 65.6% of adults authorized in a FLOC, as defined in Information Item C, which is posted at xxxxx://xxx.xxx.xxxxx.xxx/doing-business- hhs/provider-portals/behavioral-health-services-providers/behavioral-health-provider- resources/community-mental-health-contracts, shall receive at least one in-person, synchronous audiovisual, or synchronous audio-only encounter each month. FLOCs (as defined in Information Item C, which is posted at xxxxx://xxx.xxx.xxxxx.xxx/doing-business-hhs/provider-portals/behavioral-health- services-providers/behavioral-health-provider-resources/community-mental-health- contracts) included in this measure are LOC-1S, LOC-2, LOC-3, and LOC-4. LOC- 1M is excluded from this measure. Additionally, individuals who are both recommended and authorized for LOC-1S are excluded from this measure. Encounters may be for any service and for any length of time.
Adult Services. Description: Adult services are designed to support many aspects of adult life, including employment, independent and supported living, financial planning, community involvement, education, transportation, healthcare and self-advocacy. All of these areas play a role in successfully obtaining and maintaining employment.
Adult Services. County shall:
(a) Provide coordination of care services for Individuals living in residential treatment programs. The coordination of care shall include participation in the residential Provider’s treatment planning process and in planning for the Individual’s transition to outpatient services;
(b) Comply with Outpatient Services, as described in OAR 309-019-0100 through 309-019-0220, and Community Treatment and Supports, as described in OAR 309-032-0301 through 309-032-0890, as such rules may be revised from time to time; and
(c) Maintain a Certificate of Approval for the delivery of clinical services in accordance with OAR 309-008-0100 through OAR 309-008-1600, as such rules may be revised from time to time.
Adult Services. Full‐time Employees in Residential Program The regularly assigned hours shall not exceed eighty (80) in a two (2) week period. The Employer shall attempt as much as practicable to base this period on eight (8) hours per day, forty (40) hours per week. This shall include a paid meal period of one‐half (½) hour. It is understood that full‐time employees in twenty‐four (24) hour support homes must remain on the premises of the Employer during the paid one‐half (½) hour meal period and shall be required to perform such duties as are necessary during such meal period.
Adult Services. 12 a. Adult Telephone and Individual Counseling - CONTRACTOR shall provide one (1) 13 hour of individual counseling or structured telephone counseling to at least one hundred sixteen (116) 14 adult tobacco users. CONTRACTOR shall complete thirty (30) day and ninety (90) day follow-up 15 surveys.
16 b. Adult One-Time Seminar - CONTRACTOR shall provide ninety (90) minutes of 17 cessation counseling seminars to at least sixty (60) adult tobacco users. CONTRACTOR shall complete 18 thirty (30) day and ninety (90) day follow-up surveys.
19 c. Adult Sessions – CONTRACTOR shall provide a series of five (5), forty-five (45) 20 minute cessation sessions to at least seventy-nine (79) adult tobacco users. An individual must attend at 21 least three (3) cessation classes to be counted as a participant. CONTRACTOR shall complete thirty 22 (30) day and ninety (90) day follow-up surveys.
23 x. Xxxxx Adult Telephone Counseling - CONTRACTOR shall provide a series of one (1) 24 hour counseling sessions to at least fifty-six (56) young adult tobacco users, age eighteen (18) to 25 twenty–four (24) years old, after they have been given “quit kits” to assist them in quitting smoking. 26 CONTRACTOR shall complete thirty (30) day and ninety (90) day follow-up surveys.
Adult Services. Adults with learning disabilities are supported through a Joint Community Learning Disability service located within Social Work and Health but including NHS Services. The joint service includes: • Assessment and care management • Community learning disability nursing • Allied Health Professionals (Dietetics and speech and language therapy) • Day services • Respite services • Residential services Specialist services are arranged by NHS Tayside on a Tayside-wide basis. These include: • Forensic services • Challenging behaviour services. These services are located within the boundaries of Angus Council but also service Dundee City and Perth and Kinross Council areas. Service planning is undertaken by the Angus Learning Disability Planning Group and the Tayside Learning Disability Steering who report locally to the Joint Management and Commissioning Group for Community Care. Awareness of the issues surrounding the abuse of vulnerable adults is central to the delivery of learning disability services in Angus. Interim operational instructions have been developed which identify the role and responsibilities of each agency in considering interventions in relation to the alleged abuse of vulnerable adults. A consistent approach will be developed across Tayside with joint procedures to be agreed for referral and investigation of concerns.
Adult Services. A care manager or nurse from the Joint Community Learning Disability Team will assess adults with learning disabilities with community care needs. The joint team has a number or core functions that are shared such as assessment and care management but there continues to be a range of functions that are undertaken by nurses (nursing tasks) alone or social work staff (guardianship) alone. The joint team, although in its infancy, has a single management structure that aims to ensure that: • There is a single access point to specialist learning disability services; • Shared definitions, shared assessment of need and agreed service priorities are being developed; • Service users and carers have access to a mulit-disciplinary team that has a wide skill mix and has shared roles and responsibilities; • Care and support is provided in a more co-ordinated way encouraging pro-active approaches and more appropriate matching of skills and competencies; • Performance management and monitoring information can be collected within a consistent framework; • Office accommodation is shared; • The team are working to develop a local protocol which clarifies how specialist learning disability services link with generic social work and primary health care services. A single shared assessment tool and process is currently being developed to ensure a holistic approach to the identification and support of community care needs.
Adult Services a. At least seven hundred fifteen (715) adult tobacco users from Orange County will participate in an in-person individualized cessation counseling session that is sixty (60) minutes in length. Each individual counseling session will involve an initial scheduling call and a follow-up call, in addition to the thirty (30) calendar day and one hundred eighty (180) calendar day follow-up calls. Thirty three percent (33%) of the seven hundred fifteen (715) participants shall report an abstinence from tobacco use six (6) months after completing the service. No more than thirty percent (30%) of total participants shall be from Anaheim Regional Medical Center (ARMC).
b. At least four hundred eighty (480) adult tobacco users from Orange County shall participate in a one-time seminar that is one and a half (1 ½) hours in length, or two (2) seminars that are each forty-five (45) minutes in length. Each participant shall receive a scheduling call and two (2) follow–up calls at thirty (30) calendar days and one hundred eighty (180) calendar days after completing the service. Thirty-three percent (33%) of the four hundred eighty (480) participants shall report abstinence from tobacco use six (6) months after completing the service. No more than thirty percent (30%) of total participants shall be from Anaheim Regional Medical Center (ARMC).
c. At least one hundred twenty-five (125) adult tobacco users from Orange County shall participate in a series of five (5), sixty (60) minute cessation sessions. An individual must attend at least three (3) cessation classes to be counted as a participant. Each participant shall receive a follow-up call at thirty (30) and one hundred eighty (180) calendar days after completing the service. Thirty-three percent (33%) of the one hundred twenty-five (125) participants shall report abstinence from tobacco use six (6) months after completing the service. No more than thirty percent (30%) of total participants shall be from Anaheim Regional Medical Center (ARMC).
d. At least five hundred and five (505) adult tobacco users from Orange County shall participate in a series of either five (5) cessation classes, one time seminar that is one and a half (1 ½) hours in length, or two (2) seminars that are each forty-five (45) minutes in length, that focuses on those adults living in sober living facilities or treatment facilities. An individual must attend all seminar sessions, or at least three (3) of the five (5) cessation classes to be counted a...
