Performance Requirements of HSP. 2.8.1. The HSP by its relevant personnel, including but not limited to its Medical Director and patient’s attending physician, shall develop a plan of care for each patient (“Plan of Care”) for the management and palliation of the patient’s terminal illness at the time the eligible patient is admitted to the HSP program.

  • Quarterly Contractor Performance Reporting Customers shall complete a Contractor Performance Survey (Exhibit I) for each Contractor on a Quarterly basis. The completed Contractor Performance Survey(s) will be used by the Department as a performance-reporting tool to measure the performance of Contractors.
