Common use of Person-Centered Service Plans Clause in Contracts

Person-Centered Service Plans. The CHC-MCO must develop and implement a written, holistic PSCP for each Participant who requires LTSS. The CHC-MCO must comply with the PCSP requirements specified in 42 C.F.R. § 438.208(c)(3) and § 441.301(b) and (c) in developing the PCSP. The developer of the PCSP must be trained in person- centered planning using a person-centered process. Refer to Exhibit Z Person- Centered Service Planning for additional information on PCSP requirements. The PCSP must address how the Participant’s physical, cognitive, and behavioral health needs will be managed, including how Medicare coverage (if the Participant is Dual Eligible) will be coordinated and how the Participant’s LTSS services will be coordinated. The holistic PCSP at a minimum, must include the following:
