Personal Health Records. “Personal Health Records” means electronic records of personal health information, regardless of whether the information has been created or received by Provider, health plan, employer, or health care clearinghouse, in order to distinguish it from individually identifiable health information that is created or received by Provider, health plan, employer, or health care clearinghouse. Personal Health Records includes the kinds of records managed, shared and controlled by or primarily for the Patient, but not records managed by or primarily for commercial enterprises, such as life insurance companies.

  • Health Records Provider agrees to cooperate with Subcontractor and/or Health Plan to maintain and share a health record of all services provided to a Covered Person, as appropriate and in accordance with applicable laws, regulations and professional standards.

  • Personnel Records (A) There shall be only one official personnel file for each employee, which shall be maintained by the employing agency. Information in an employee’s official personnel file may be maintained in electronic as well as paper form.
