Common use of Physician’s Certificate of Recovery and Fitness Clause in Contracts

Physician’s Certificate of Recovery and Fitness. The Fire Chief or his designee may require a certificate of recovery and fitness for duty signed by a physician from an employee upon return to work from any illness that required the use of sick leave for a period longer than two (2) shifts. The Fire Chief or his designee may also require that an employee submit a certificate of recovery and fitness for duty signed by a physician in the event the employee has six (6) incidents of sick leave usage in a twelve (12) month period.
