PREFACE. The following are the special terms and conditions (STCs) for Arizona’s section 1115(a) Medicaid demonstration extension (hereinafter “demonstration”). The parties to this agreement are the state of Arizona and the Centers for Medicare & Medicaid Services (CMS). This demonstration is approved for a 5-year period, from October 22, 2011, through September 30, 2016. The STCs set forth below and the lists of waivers and expenditure authorities are incorporated in their entirety into the letter approving the demonstration. The STCs are effective as of October 22, 2011, unless otherwise specified. The STCs have been arranged into the following subject areas: I. Preface; II. Program Overview and Historical Context; III. General Program Requirements; IV. Eligibility; V. Demonstration Programs; VI. Funding Pools Under the Demonstration; VII. Delivery Systems; VIII. Evaluation; IX. General Reporting Requirements; X. General Financial Requirements under Title XIX; XI. General Financial Requirements under Title XXI; XII. Monitoring Budget Neutrality; and XIII. Schedule of State Deliverables during the Demonstration.
