Premium Payment With Separation Date. During First Pay Period Of The Month In the event an employee separates from Library Authority employment during the first pay period of the month, the health premium that would have been deducted from the second paycheck of the month shall instead be deducted from any remaining leave balance payment due to the employee upon separation. In the event the remaining leave balance is insufficient to cover the entire amount of the remaining premium due, it is the responsibility of the individual employee to pay for the remaining amount. This payment may be accomplished through deduction from final pay, or by personal check made payable to the Sacramento Public Library Authority.
