Preparation for Testing. Pursuant to Department of Transportation regulations, the City reserves the right to utilize on site or off site collection facilities. Upon arrival at the collection site, an employee must provide the collection agent with: - Photo identification. - An unsigned authorization form for urinalysis drug screening. If the employee arrives without the above-listed items, the collection agent should contact the City Safety and Health manager or City Human Resources manager. The employee signs the consent form and the collection agent signs as a witness. A standard DOT approved urine custody and control form will be supplied by the appropriate laboratory. This form must be used by all collection facilities.
