Preventative Care Dental Services. Claims for covered dependents under age 18 for oral examination and fluoride treatment are reimbursed at 100% and are not subject to the Benefit Year dollar amount if these services are not covered under the employee’s health insurance plan. The following are excluded: (1) Any service or supplies not furnished by a licensed dentist. (2) Any service or supply not reasonably necessary for the dental care of the eligible individual.
