Procedure for Requesting FMLA Leave. A teacher is required to provide thirty (30) days advance notice to take FMLA leave when the need is foreseeable in a written letter of request. If the leave is unexpected than the notice must be provided as soon as possible. It is the responsibility of the teacher to provide sufficient information to the Superintendent to determine that they will qualify for FMLA. Information should include certification from a doctor and the amount of time being requested.
