Common use of Proof of Eligibility Clause in Contracts

Proof of Eligibility. (A) MCHCP reserves the right to request proof of eligibility at any time. If such proof is not received or is unacceptable as determined by MCHCP, coverage for the applicable dependent or spouse/child(ren) will be termi- nated or will not take effect.
