Common use of PROPOSAL FORMS Clause in Contracts

PROPOSAL FORMS. At the request of a prequalified Xxxxxx, the Contracting Agency will provide a Proposal Form for any project on which the Bidder is eligible to Bid. The Proposal Form will identify the project and its location and describe the Work. It will also list estimated quantities, units of measurement, the items of Work, and the materials to be furnished at the unit Bid prices. The Bidder shall complete spaces on the Proposal Form that call for unit prices, extensions, the total Bid amount, signatures, date, acknowledgment of Addenda, and the Bidder’s address. The required certifications are included as part of the Proposal Form.
