Provisional Plan of Care Sample Clauses

Provisional Plan of Care. The parties agree to work in collaboration to develop a mutually agreed upon process to create a provisional plan of care process by October 31, 2019 for the following: Crisis requests, Exceptions requests and I/DD waiting list offers for waiver services through KDADS. The parties acknowledged these provisional plans of care do not apply to annual reassessments and shall include relevant State Plan services and must include at least one waiver service. The parties acknowledge the completion of this provisional plan of care by the CDDO shall in no way shift any case management functions or service from the community system to the MCOs. The parties acknowledge, material changes to the scope of work for the CDDO would be subject to remedies noted in this contract.
