Requests for Accountings. Business Associate agrees to provide to Covered Entity or an Individual, in the time and manner reasonably requested by Covered Entity, information collected in accordance with Section 2.k. of this Agreement, to permit Covered Entity to respond to a request by an Individual for an accounting of disclosures of Protected Health Information in accordance with HIPAA. If an Individual makes a request for an accounting directly to Business Associate, Business Associate shall notify Covered Entity of the request within three (3) business days of such request and will cooperate with Covered Entity and allow Covered Entity to send the response to the Individual.
Requests for Accountings. Business Associate agrees to provide to Covered Entity, within twenty (20) days of a request by Covered Entity, information collected in accordance with Article II, Section 9 of this BA Agreement, to permit Covered Entity to respond to a request by an Individual for an accounting of disclosures of Protected Health Information in accordance with HIPAA, HIPAA Regulations and the HITECH Act. If an Individual makes a request for an accounting directly to Business Associate, Business Associate shall notify Covered Entity of the request within three (3) business days of such request and will cooperate with Covered Entity and allow Covered Entity to send the response to the Individual.

Subcontractor agrees to provide to Business Associate, or at the direction of Business Associate, to a Covered Entity or an Individual, within fifteen (15) days of a request by Business Associate, information collected in accordance with Section 2(j) of this Agreement, to permit a Covered Entity or Business Associate to respond to a request by an Individual for an accounting of disclosures of Protected Health Information in accordance with HIPAA, HIPAA Regulations and the HITECH Act. If an Individual makes a request for an accounting directly to Subcontractor, Subcontractor shall notify Business Associate of the request within three (3) business days of such request and will cooperate with Business Associate and any Covered Entity and allow Business Associate or the Covered Entity to send the response to the Individual.

Business Associate Subcontractor agrees to provide to Business Associate, in the time and manner reasonably requested by Business Associate, information collected in accordance with Section 3.h. of this Agreement, to permit Business Associate to respond to a request by an Individual for an accounting of disclosures of PHI.

Business Associate agrees to provide to Covered Entity or an Individual, in the time and manner designated by the Covered Entity, information collected in accordance with Section 2(i) of this BA Agreement, to permit Covered Entity to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with HIPAA, HIPAA Regulations and the HITECH Act as of its effective date. If an Individual makes a request for an accounting directly to Business Associate, Business Associate shall notify Covered Entity of the request within ten (10) business days of such request and will cooperate with Covered Entity and allow Covered Entity to send the response to the Individual.

Business Associate agrees to provide to Covered Entity, within thirty (30) days of a request by Covered Entity, information to permit Covered Entity to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with HIPAA, HIPAA Regulations and the HITECH Act.If an Individual makes a request for an accounting directly to Business Associate, Business Associate shall notify Covered Entity of the request within three (3) business days of such request and will cooperate with Covered Entity and allow Covered Entity to send the response to the Individual.

Business Associate agrees to provide to HMH or an Individual, in the time and manner designated by the HMH, information collected in accordance with Section D(10) of this BAA as needed by HMH to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with HIPAA and the HITECH Act. If an Individual makes a request for an accounting directly to Business Associate, Business Associate shall notify HMH of the request within three (3) business days of such request and will cooperate with HMH and allow HMH to send the response to the Individual.
Requests for Accountings. Business Associate agrees to provide to Covered Entity for an Individual, in the time and manner reasonably requested by Covered Entity, information collected in accordance with Section 5.k. of this Agreement, to permit Covered Entity to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with HIPAA. If an Individual makes a request for an accounting of disclosures of PHI directly to Business Associate, Business Associate shall notify Covered Entity of the request within three (3) business days of such request and will cooperate with Covered Entity and allow Covered Entity to send the response to the Individual. The duty of the Business Associate and its agents and subcontractors to assist Covered Entity with any HIPAA required accounting of disclosures survives the termination of the Contract.
