Risk Stratification Levels. The MCP must develop a strategy to assign members to low, medium, or high risk stratification levels based on the results of the identification and/or assessment processes. This will be a continual process and the risk levels will be adjusted based on the completion of the health assessment and the member’s demonstrated progress in meeting the goals of the care treatment plan. Each MCP must incorporate risk stratification levels as specified in ODJFS Care Management Program Requirements.
