SBA Forms. Completed SBA Forms 480, 652, and 1031 (Parts A and B), showing Borrower’s financial projections (including balance sheets and income and cash flow statements) for the period described therein and a representation to PIOP of Borrower’s intended use of proceeds of the Loans (the “Use of Proceeds Statement”).

SBA Forms. The Company shall have executed and delivered: a Size Status Declaration on Small Business Administration ("SBA") Form 480, an Assurance of Compliance on SBA Form 652, a Portfolio Financing Report on SBA Form 1031, and such other forms as may be necessary or desirable under applicable SBA rules and regulations in order to enable MidMark to complete its investment in the Company.
SBA Forms. Parent shall have executed and delivered to Lenders SBA Form 1031, PART A of which shall have been completed by Parent, and SBA Forms 652-D and 480.
SBA Forms. Upon the request of SG, the Company shall complete, execute (where applicable) and deliver to SG, SBA Forms 480, 642 and 1031.
SBA Forms. The Company shall deliver to CVC prior to the Closing:
(a) duly completed and executed SBA Forms 480, 652, and Part A of 1031;
(b) a business plan showing the Company's financial projections (including balance sheets and income and cash flow statements) for the period ending December 31, 1997;
(c) a written statement from the Company regarding its intended use of the proceeds of the purchase of Stock hereunder; and
(d) a list, after giving effect to the transactions contemplated by this Agreement, of (a) the name of each of the Company's directors, (b) the name and title of each of the Company's officers, and (c) the name of each of the Company's stockholders setting forth the number and class of shares held.
SBA Forms. Small Business Administration Forms 480, 652 and 1031, duly completed at or promptly following the Initial Closing Date, upon the consent of MVP II.
SBA Forms. The Company shall have executed each of the Small Business Administration forms referenced in Section 2.6 and the Investor shall have received copies of such executed forms.
SBA Forms. Lender shall have received executed and completed SBA form 480, 652, and 1031, each in form and substance acceptable to Lender.
