SEP Plan. The person’s BEAUTIFUL HEARTS AND MINDS Supported Employment Program (SEP) plan shall be developed consistent with the consumer’s IPE in terms of the employment goal, work hours per week and other relevant factors. It must contain the description of the job development (if applicable), natural supports available, and integrated work setting services to be provided; provision for periodic monitoring to ensure progress towards meeting the goal and a description of the extended services.
