Special Incident Reporting. CONTRACTOR shall provide written notification to COUNTY of any critical incidents and outcomes that may have occurred at the COUNTY-owned facility, or to the staff or patients under CONTRACTOR’S jurisdiction. CONTRACTOR shall prepare a COUNTY Health and Human Services Department “Incident Report” form in accordance with Placer County Health and Human Services, Adult System of Care Policy and Procedures.
