CONTRACTOR shall provide reports as requested by COUNTY including but not limited elements identified in the Practice Guidelines, Quality Assurance Work Plan/Quality Improvement Work Plan, Compliance Plan, Cultural Competence Plan, or any standard measures required by DHCS and any data specified that enables COUNTY to measure the CONTRACTOR’S performance. REPORTING AND EVALUATION REQUIREMENTS. 31.1. Grantee shall submit the following reports to the Grantor on a monthly basis: • progress reports for the preceding month relevant to the performance indicators listed in the Agreement; • fiscal reports detailing financial expenditures for the previous month; and • any other reports specified by the Grantor. Reports shall be submitted by the 15th of every month following the first complete month of the grant period. 31.2. Grantee is further required to submit a final financial status report following termination of the program, the content and form of which will be determined by the Executive Director of the Authority. 31.3. Xxxxxxx agrees to comply with the Grantor’s request for information related to an evaluation of program. The Grantee agrees to report any additional information required by the Executive Director of the Authority. 31.4. Grantor may give the grantee permission, in writing, to report on a quarterly schedule. Such permission can be revoked by the grantor at any time. If such permission is given, the quarterly reports should be submitted based on the following schedule: Quarter End Date Due Date September 30 October 15 December 31 January 15 March 31 April 15 June 30 July 15

CONTRACTOR shall track the number and type of complaints received during the year by its agency for review by its staff, and COUNTY CSOC leadership and Quality Improvement staff.

Quarterly Reporting/Audit Requirements: The CONTRACTOR shall submit quarterly reports indicating open cases, as well as services provided. The report shall also include a synopsis of all cases that closed in the prior quarter, as well as the level of client and family functioning upon closure. The CONTRACTOR shall make charts available for Placer County and/or State and Federal auditors to audit for Medi-Cal regulation compliance, upon notice. Notice may be as little as 24 hours in the case of a State or Federal audit, including EPSDT and DHCS audits. The CONTRACTOR will also perform internal monthly chart audit activities with a report of such activities sent to the County with the quarterly report of open cases and services provided.

REPORTING AND EVALUATION REQUIREMENTS. Maintaining credibility for the community development effort rests heavily on the ability to produce an impact in income-eligible areas, through progress in accomplishing scheduled activities. An effective method for maintaining program progress against a previously established schedule is through Program evaluation and reporting, which will consist of both written reports and staff discussions on a regular basis. As required by HUD, the City will conduct monitoring visits with the Consultant as needed. 19.1 Monthly Reports are due no later than the fifteenth (15th) day of the succeeding month and shall include the request for payment when applicable. Contents of the Monthly Report, shall include but not necessarily be limited to, the following: 1. The Narrative Report; 2. The Financial Status Report, which shall include the request for payment and documentation, as applicable;

CONTRACTOR shall prepare a COUNTY Health and Human Services DepartmentIncident Report” form in accordance with Placer County Health and Human Services, Adult System of Care Policy and Procedures. 8.1.3. CONTRACTOR will work in partnership with COUNTY and COUNTY sub-contracted Evaluation Firm, Indigo Project, to gather and provide any data requested. This evaluation process will continue throughout the duration of this agreement and may include but is not limited to submission of data and reports, interviews of CONTRACTOR staff and leadership, participation in on-going meetings.
