CONTRACTOR shall provide written notification to COUNTY of any critical incidents and outcomes that may have occurred at their facilities, County- owned facilities, or to the staff or patients under CONTRACTOR’S jurisdiction. CONTRACTOR shall prepare a report utilizing the California Department of Social Services “Unusual Incident/Injury Report” form in accordance with the California Department of Social Services Community Care Licensing Division.
