Submit Repair Orders for Payment. Once authorization is obtained and the repair is completed, all repair orders and documentation must be submitted to the ADMINISTRATOR, at the address shown below, within sixty (60) days from the date the covered repair was completed to be eligible for payment. The documentation must include the following: current odometer mileage; AGREEMENT number; complaint, cause of failure and corrective action; cost of the repair; the last eight (8) digits of the VEHICLE identification number; REPAIR FACILITY’S phone number and AGREEMENT holder’s signature.
