Common use of Surviving Spouse Clause in Contracts

Surviving Spouse. (a) For Calendar Years 2018, 2019, and 2020 the provisions of subsection 3.c. (1) shall be applicable to a surviving spouse eligible d. Duty Death (1) (a); in all other circumstances he/she shall be covered by subsection 3.c.(1)(b). If the subscriber cost for enrollment in the plan elected exceeds the maximum City contribution provided, the employee shall have the amount of excess cost deducted from his/her pay check on a monthly basis.
