Vaccine Passport Prohibition. Service Provider certifies that it does not require its customers to provide any documentation certifying the customer’s COVID-19 vaccination or post-transmission recovery on entry to, to gain access to, or to receive service from the Service Provider’s business. Service Provider acknowledges that such a vaccine or recovery requirement would make Service Provider ineligible for a state-funded contract.

A/E acknowledges that such a vaccine or recovery requirement would make A/E ineligible for a state-funded contact and shall be grounds for termination of this Agreement for cause.
