COVID-19 Vaccination. The Company requires COVID-19 vaccinations as a condition of employment. If you have any questions about COVID-19 vaccinations, you should consult a trusted healthcare provider. Unless an exemption applies, the Company requires that all employees be fully vaccinated before their Start Date. An employee is considered “fully vaccinated” if at least 14 days or a period specified by the vaccine’s manufacturer have elapsed since the employee received the last dose of a vaccine that the FDA has authorized for use in the United States. FDA-authorized vaccinations include vaccinations that have been authorized pursuant to an Emergency Use Authorization. For an employee fully vaccinated outside of the United States, the vaccination must be listed for emergency use by the World Health Organization (“WHO”). To establish that you are fully vaccinated, we require that you provide evidence of immunization by presenting a completed COVID-19 Vaccine Card or documentation from an authorized healthcare provider or pharmacy proving that you have received the COVID-19 vaccine. Please submit these documents to Xxxxxxx Xxxxxxxx at or via the Company’s HR portal. The Company must receive this documentation prior to your projected Start Date. Employees who have i) a qualifying medical condition that contraindicates the vaccination, or ii) who object to being vaccinated on the basis of a sincerely held religious belief or practice, may contact Xxxxxxxx Xxxx at . COVID-19 Vaccination. Except as otherwise provided below, to the extent this Contract requires Mechanical Engineer, employees of Mechanical Engineer, or any subcontractor to perform any type of in-person work, including, but not limited to, the practice of medicine and health care, consulting, training, construction, maintenance, and vendor services, on property owned or leased in any capacity by CSN, any and all employees of Mechanical Engineer and each subcontractor who will perform such in-person work must have completed a COVID-19 vaccination series before they will be permitted to perform such in-person work. By executing this Contract, Mechanical Engineer represents and warrants that it is solely responsible for ensuring full compliance with this provision and such compliance shall occur before any Mechanical Engineer or subcontractor employee covered by this provision enters CSN property. Failure to comply with the foregoing shall constitute a material breach of this Contract, which, if not fully cured within thirty (30) days after CSN’s written notice of breach, entitles CSN to terminate the Contract. No inability, delay, or failure to comply with this provision shall be deemed a valid excuse or justification for Mechanical Engineer to fail to timely complete the work specified herein. A “completed COVID-19 vaccination series” is given the same meaning as in Title 2, Chapter 12, Section 4(C) of the Nevada System of Higher Education Code and is defined below and made a part of this Contract. Within 72 hours of CSN’s written request to Mechanical Engineer, Mechanical Engineer shall provide to CSN records, acceptable to CSN in its sole discretion, of a completed COVID-19 vaccination series for any and all employees of Mechanical Engineer and each subcontractor performing such in-person work. Notwithstanding anything to the contrary contained above, the foregoing vaccination requirements shall not apply to any employee of Mechanical Engineer/subcontractor working on a construction project which is not reasonably accessible to CSN employees or students, or members of the public, provided, however, that such employees shall comply with all directives of the Centers for Disease Control and Prevention, Occupational Health and Safety Administration, and the state of Nevada concerning COVID-19. COVID-19 Vaccination. 1. The Board of Trustees has mandated that all faculty members who work on site must be fully vaccinated against COVID-19, unless an exception is granted for a medical or religious reason. Faculty members who have a medical or religious reason must notify the District to provide documentation supporting their status.
COVID-19 Vaccination. It is the duty of every employer to protect the health and safety of employees by establishing and maintaining a healthy and safe work environment and by requiring all employees to comply with health and safety measures. All employees are required required to complete their primary series of COVID-19 vaccines (e.g. be fully vaccinated, does not include boosters) according to the schedule recommended by the U.S. Centers for Disease Control and Prevention, or be approved by the employer for medical or religious exemption and accommodation, as a condition of employment. Employees who fail to maintain this condition of employment for their position will be subject discipline, up to and including termination.
