Contractor’s Certification. I certify that I understand the training requirements above and will ensure that I and all of my agents and employees who may or will have direct, unsupervised contact with students will take all of the identified trainings before beginning work under this Contract. I understand that it is my responsibility to monitor training progress of my agents and employees and ensure training completion before work begins. I understand that Portland Public Schools may from time to time request records indicating training completion, and I will be prepared to provide those records immediately upon request. Contractor’s Certification. By entering into this contract, the Contractor (and officials thereof) certifies that neither it (nor he or she) nor any person or firm who has a substantial interest in the Contractor's firm is a person or firm ineligible to be awarded District contracts by virtue of the sanctions imposed under section 5 of the Act. No part of this contract shall be subcontracted to any person or firm ineligible for award of a District contract under section 5 of the Act. The penalty for making false statements is prescribed in the U.S. Criminal Code, 18 U.S.C. §1001.

Contractor’s Certification. Each Application for Payment shall be accompanied by the Contractor's signed certification that all amounts paid to the Contractor on the previous Application for Payment that were attributable to Subcontractor Work or to materials or equipment being supplied by any supplier has been paid over to the appropriate Subcontractors and suppliers; that all amounts currently sought for Subcontractor Work or supply of materials or equipment are currently due and owing to the Subcontractors and material or equipment suppliers; and that all Work, materials or equipment for which payment is sought is, to the best of the Contractor's knowledge, free from defect and meets all of the Contract requirements. The Contractor shall not include in an Application Payment amounts for Work for which the Contractor does not intend to pay.
Contractor’s Certification. The Contractor certifies that it has been pre-qualified by the State of Indiana’s Public Works Division Certification Board to perform the work and furnish the services required by this Project. The Contractor further certifies that all information and documentation submitted by it in its Application for Prequalification Certification, the Contractor’s Proposal and submitted in response to the Project, is true, accurate and complete as of the date of this Contract’s effectiveness. The Contractor shall immediately notify the State of any material change to such information. The Contractor shall immediately notify the State if, during the course of performance of this Contract, it or any of its principals are proposed for debarment or ineligibility, or become debarred or declared ineligible, from entering into contracts with the federal government or any department, agency or political subdivision of the State.
Contractor’s Certification. Contractor certifies that it will comply with all applicable drug and alcohol testing requirements provided by law, including, but not limited to, the drug and alcohol testing requirements set forth in the Department of Transportation’s regulations.

Section 10.5.4. that the Contractor’s subcontracts include the clauses required by subparagraphs (1) through (4) of D.C. Official Code §2-221.02(d) (2017); and Section 10.5.5. The Contractor shall not include in an Application for Payment amounts for Work for which the Contractor does not intend to pay.
Contractor’s Certification. A bidder for a state construction contract or state assisted construction contract will not be eligible for award of the contract unless such bidder has submitted to the administering agency the following certification, which will be incorporated into the resulting contract: 1. Will not discriminate in their employment practices; 2. Intend to use the following listed construction trades in the work under the contract and 3. Will make good faith efforts to comply with the minority employee and women employee workforce participation ratio goals and specific affirmative action steps contained herein; and 4. Are in compliance with all applicable federal and state laws, rules, and regulations governing fair labor and employment practices; and
Contractor’s Certification. All applications for payment shall contain the Contractor’s certification that all his labor for the period for which payment is claimed has been paid, including all amounts to the account of such labor lawfully required to be allocated, withheld, or set aside, and that he has assured himself and represents that all labor on the account of Subcontractors or Sub-subcontractors for which payment amounts are claimed has also been paid.
