WAIVING OF MEDICAL BENEFITS. Employees may waive their opportunity to participate in the Health Insurance Plan by signing the appropriate waiver form. If an Employee is eligible for healthcare benefit coverage during Open Enrollment and does not wish to enroll, the Employee must sign a waiver stating same.
