Health, Dental, and Vision Benefits. Hospital and health insurance benefits are offered to Trainees and their eligible dependents with available coverage from the first recognized day of training. It is the Trainee's obligation to select and enroll in the benefit program(s) he/she desires;
Health, Dental, and Vision Benefits. 1. Employees shall be eligible for the following health, dental, and vision benefits upon acceptance of written applications by the benefit carrier and shall be effective when the requirements established by the benefit carrier are met.
2. An Employee who is eligible for the health care insurance provided by this Section shall not be entitled to receive such insurance coverage if they are eligible to receive such insurance coverage through another employer, with the following exceptions:
(1) the health care double coverage prohibition shall not apply if the other employer will not permit its employees to drop the health insurance coverage provided through the other employer for the Employee or the Employee's family members and so states in writing to the Board, and (2) an Employee who has family members who receive health insurance coverage as a result of a divorce decree shall be entitled to receive the health care insurance provided by this Section for those eligible family members who do not receive health care insurance as a result of the divorce decree.
3. The Employee will be eligible for Board-paid dental care insurance only if they do not receive such dental care insurance coverage through another employer. For the appropriate coverage, the Employee shall verify in writing that they are eligible for such coverage. Written verification shall be completed at the beginning of each school year. After that time any Employee with double health or dental coverage shall reimburse the Board the cost of their health or dental coverage for the duration of such double coverage.
4. Any changes in family status shall be reported by the Employee in writing to Human Resources within thirty (30) days of such changes. The Employee shall be responsible for any overpayment of premiums by the Board on their behalf for failure to comply with this provision.
5. Health benefits:
a. The health benefit plan design shall be comparable to that offered to the XXXXX employee group.
b. The Board shall contribute a maximum monthly amount equivalent to the State hard caps towards health benefits for qualified employees in accordance with MCL15.563. Such amounts are subject to adjustment by the State Treasurer annually.
c. Board contribution will be pro-rated based upon FTE for part time employees.
Health, Dental, and Vision Benefits. The Village shall grant employees covered by this Agreement, except as specifically and expressly limited by this Agreement, all benefits and provisions including but not limited to the following: life, health, dental and vision insurance, vacation leave, special leave and uniform provisions as granted under the Buffalo Grove Personnel Rules as set forth on June 1, 1984; and changed thereafter.
Health, Dental, and Vision Benefits. 7.2.1 Each eligible member shall enroll in one of the health insurance plans offered under CalPERS. Information on the CalPERS plans, including benefits, coverage limitations, deductibles, copays, and coinsurance, is available online at xxxx://xx.xxxx.xxx/benefits/_medical-revised.html. Information for Plan Year 2024 is available in Appendix J.
7.2.2 Opt Out
Health, Dental, and Vision Benefits. The Employer will provide employees with coverage under the Blue Cross/Blue Shield Plan as amended from time to time, however, the employer reserves the right to self-insure or to change carriers as it deems appropriate, provided that the coverage and benefits remain substantially the same. Any employee wishing to waive the health benefits may do so by signing and filing the appropriate form in the Human Resources Department. The Employer and the active employees shall share the cost of health, vision and dental coverage as follows: PPO Employer % Employee % Single 90% 10% EE+1 80% 20% EE+2 or more 80% 20% HMO Employer Employee Single 90% 10% EE+I 86% 14% EE+2 or more 85% 15% Effective 1/1/2016, the Employer will make available to employees the Employer’s 2016 PPO Plan design. Effective 4/11/17, the Employer will make available to employees the Employer’s 2017 HMO Plan design. Co-pay reimbursements will cease on May 2. The dollar amount of employee contributions will be adjusted on the renewal date, January 1 of each calendar year based upon the cost to the Employer and the cost sharing percentages set forth above. Bargaining unit employees, employed on 4/11/17, shall receive a one-time $100 wellness 2017 bonus to be paid by separate check on May 19, .
Health, Dental, and Vision Benefits. Unit members on unpaid leave may choose to retain and pay for health and welfare benefits, subject to the insurance carrier’s approval. The unit member on leave shall pay the total monthly premium and send the remittance to the District Office by the fifth working day of each month. The District will also accept quarterly premium payments paid in advance.
Health, Dental, and Vision Benefits. A. Consolidated Benefits (CoBen) Program Description
1. CoBen Allowance Effective on the first day of the pay period following ratification of this agreement, the State will continue to pay the contribution rates established on January 1, 2013, for the Consolidated Benefits (CoBen) allowance. The allowance is based on the Health Benefit party codes in a health plan administered or approved by CalPERS. To be eligible for this contribution, an employee must positively enroll in a health plan administered or approved by CalPERS.
a. The State shall contribute $544 per month for coverage of an eligible employee. (Party code one).
b. The State shall contribute $1,073 per month for coverage of an eligible employee plus one dependent. (Party code two).
c. The State shall contribute $1,402 per month for coverage of an eligible employee plus two or more dependents. (Party code three). The employer health benefits contribution for each employee shall be an amount equal to eighty (80%) percent of the weighted average of the Basic health benefit plan premiums for a State active civil service employee enrolled for self-alone, during the benefit year to which the formula is applied, for the four Basic health benefit plans that had the largest State active civil service enrollment, excluding family members, during the previous benefit year. For each employee with enrolled family members, the employer shall contribute an additional eighty (80%) percent of the weighted average of the additional premiums required for enrollment of those family members, during the benefit year to which the formula is applied, in the four Basic health benefit plans that had the largest State active civil service enrollment, excluding family members, during the previous benefit year. The Union agrees to support legislation to amend Government Code Section 22871.8 regarding the employer’s share of the employee health contribution as described above from 85% to 80%. When an employee is appointed to a new position or class that results in a change in eligibility for the composite rate, the effective date of the change shall be the first of the month following the date the notification is received by the State Controller's Office if the notice is received by the tenth of the month.
Health, Dental, and Vision Benefits. Contribution
A. Effective July 1, 1999, Health, Dental, and Vision monthly contributions to the California Stationary Engineers, Unit 13 Health and Welfare Trust Fund shall be $420 for each eligible Unit 13 employee and annuitant.
B. Effective 1/1/2000, said contribution shall be $440 for each eligible Unit 13 employee and annuitant.
1. For purposes of this Agreement, eligible employee and annuitant are defined the same as in the Public Employees' Medical and Hospital Care Act of the Public Employees' Retirement Law.
2. Any annuitant who, at the time he/she became an annuitant, was enrolled in the California Stationary Engineers, Unit 13 Health and Welfare Trust Fund, may continue his/her enrollment without discrimination as to premium rates or benefit coverage.
3. Any surviving spouse of a Unit 13 employee who is eligible for retirement has the same benefit election rights of a surviving spouse in a PERS Health Plan.
C. The specific details of the California Stationary Engineers, Unit 13 Health and Welfare Trust Fund are found in Addendum "A" of this Agreement.
D. Deductions for this plan shall be made in accordance with the State Controller's Office administrative procedures.
E. Notwithstanding any provisions of the IUOE Health and Welfare Trust or any decision on the part of the trustees of said trust to increase the employer's contribution to the IUOE Health and Welfare Trust, the Union recognizes that the State's maximum contribution to the Trust, for the duration of this contract, shall not exceed the amounts referenced in 10.1 A & B, above. Benefit Review
A. The parties agree that, if during the term of this Agreement, it is no longer feasible to provide health, dental, vision and other benefits that are currently provided by the Operating Engineers’ Trust Plan, the Union and the State will jointly sponsor legislation to provide health benefit coverage to all employees and retirees in Bargaining Unit 13 pursuant to the Public Employees’ Medical and Hospital Care Act.
B. The parties further agree that if the employees and retirees become covered under PEMHCA, dental benefits for both active and retired employees will be provided by the Department of Personnel Administration. Vision benefits for active employees will be provided by the Department of Personnel Administration.
Health, Dental, and Vision Benefits. The District shall continue to provide the current benefit package (Health, Dental, Vision insurance) to all employees who are covered by the terms of this agreement, and their eligible dependents. The District’s maximum yearly contribution shall be $16,843 for 12 month employees. Employees who work less than 12 months a year will receive a prorated District contribution based upon how many months they work. Any costs above the maximum District contribution shall be borne by the individual employee. (7/1/08) While the level of coverage in the benefit package is subject to negotiations, the provider is at the discretion of the District, (4-11-90) Both parties agree to coordinate a meeting between classified employees and the insurance provider to consider changes to the insurance benefits plan. Health, Dental, and Vision Benefits. 15.2.1 Each eligible supervisor shall enroll in one of the health insurance plans offered under XxxXXXX. Information on the CalPERS plans, including benefits, coverage limitations, deductibles, copays, and coinsurance, is available online at xxxx://xx.xxxx.xxx/benefits/_medical-revised.html. Information for Plan Year 2019 is available in Appendix G.
15.2.1.1 The District shall provide dental care benefits for each eligible supervisor and his or her eligible dependents.
15.2.1.2 The District shall provide vision care benefits for each eligible supervisor and his or her eligible dependents.
15.2.2 Opt Out
15.2.2.1 Supervisors may elect to waive coverage in accordance with the District’s open enrollment process for the subsequent plan year.
15.2.2.1 An opt-out election shall remain in effect during the entire Plan Year, and the supervisor may not re-enroll in a CalPERS plan except during Open Enrollment or as a consequence of an IRS Section 125 qualifying event. Waiver of coverage shall not result in a compensated allowance in lieu of coverage.
