The Service Provider must maintain a webpage dedicated to the SSA which is easily identified with a link labeled "SSA" on the top or side banner on the home page of the Service Provider's website. The SSA webpage must contain the following information: • Listing of Commission meetings for the year • Meeting Notices and Agendas posted at least 48 hours prior to meetings • Approved Meeting Minutes • Current Service Provider Agreement • Annual Audit (most-recent)Request for Proposals (RFPs) • SSA Map • Description of Services and Programs • Other items as directed by the Commission Reports: The Service Provider is required to complete and submit Quarterly Reports to the Department of Planning and Development (DPD) within 30 days of the end of each Quarter. These reports should be presented to and approved by the Commission prior to submission to DPD.
