(4) No further benefits For the purpose of determination of what an “illness” is and what an “accident” is under Article 26.02, the treating doctor will be payable provided with a form which will include the definitions of both terms from the date you refuse Group Benefit Plan and the Company and Union agree that the decision of the doctor as evidenced on that form will be the deciding factor in the determination of accident or illness for the purpose of the waiting period.
(5) Successive periods of disability of an employee separated by less than four (4) weeks of active full-time employment at the employees’ regular job shall be considered one continuous period of disability unless the subsequent disability is due to participate an injury or sickness unrelated to the causes of the previous disability and begins after the employee has returned to active full-time employment.
(6) It will be the responsibility of the employee to submit appropriate information concerning his/her disability on the form designated by the Company before Weekly Indemnity Benefits become payable. The Company is prepared to reimburse $50.00 for any WI documents that are approved and the employee submits a receipt. This is valid only for one time per contract.
(7) No weekly indemnity benefits shall be payable for any period of disability during with the employee is not wholly, and continuously disabled and is not under the treatment of a physician legally licensed to practice medicine.
(8) The Company shall provide an employee who is on WSIB or Weekly Indemnity after 16 weeks with the appropriate LTD forms. The company will file the forms on behalf of the employee within the applicable time limits provided the employee returns the required documents in a rehabilitation program timely manner.
(9) In the event of the death of an employee the benefit plan coverage for eligible dependents will be continued for two (2) months following the month in which is considered appropriate by the Insurance Companydeath occurs.

Weekly Indemnity. (iEffective on the first of the month following the signing of this Supplemental Agreement, for claims which originate on or after that date, an eligible employee will be entitled to weekly indemnity benefits of of weekly base pay to a maximum of per week. (i) Effective date of ratification of this Agreement, for those employees at work, the Weekly Indemnity - 66 2/3coverage will be 70% of your weekly salary payable to one Job Box lower than the employee’s incumbency rate times 40 hours per week beginning on the first day in the case of an accident or hospitalization and on the fourth day for sickness (other than accident or sickness compensable under the Worker’s Safety and Insurance Board of the province of Ontario) and continuing throughout the disability for a maximum weekly benefit of $650.00 per week for 26 52 weeks. Benefits begin “Accident” will be defined as a sudden unforeseen event, definite as to time and place causing injury to the physical structure of the body, resulting from an external agent or trauma (as opposed to harm resulting from disease), happening involuntarily or if the result of a voluntary act entailing unforeseen consequences. If, during the term of the Agreement, the sickness benefit payable under the Employment Insurance Act or regulations thereto exceeds the amount of weekly indemnity under the Insurance Program, the amount of Weekly Indemnity will be adjusted so as to equal the amount payable under the Act. Any increase in premiums during the life of the Agreement will be deducted from the contingency fund. For those employees not at work at the time of a change in the amount of Weekly Indemnity, such coverage will be effective upon their return to work. Certification of disability by a chiropractor or osteopath will be accepted for up to four (4) weeks from commencement of disability. Should the Company qualify for a rebate under the Unemployment Insurance Act with respect to the Weekly indemnity Benefits, the “5/12” portion will be assigned to the contingency fund. Income tax will be deducted from Weekly Indemnity payments. This is not optional. The cost to provide medical verification will be borne by the employee. As an alternative to having an employee off work to get medical verification, the Company will review each case on an individual basis, discuss the 1st day if disability is due to injury situation with the Union President and examine placing the 4th day if disability is due to sicknessemployee in a suitable controlled environment position. If hospitalized, benefits will begin on the 1st day.
(ii) Benefits will not be paid beyond: □ the 26th week from the date of disability; or □ the date you cease to be totally disabled; or □ the date of your 65th birthday; or □ retirement or the date you withdraw or elect to receive pension funds; or □ the date you engage in any work or occupation; or □ the date you fail to furnish satisfactory evidence of total disability or refuse to submit to a medical examination by a physician chosen claim is accepted by the Insurance Company; whichever first occurs.
(iii) List of Exclusions
(1) any period of disability Carrier and additional documents from a Specialist is required by The Workers’ Compensation Act or any similar legislation. A committee consisting of the Manager of Human Resources, H.R. Generalist along with the Union President and one union designate will make up the members of the committee. This committee will be responsible to apply for Group Insurance benefits while find a resolution to any situation that may arise from time to time with respect to Great West Life’s adjudicating an employees claim. The committee will determine the employee’s claim is under appeal to the Saskatchewan Workersappropriate course of accommodation utilizing Doctors’ Compensation Board. If the appeal is successfulreport, the employee shall be solely responsible for repayment of Function Ability Form and any benefits claimed under the Group Insurance Plan.
(2) any disability resulting from or caused by: □ intentionally self-inflicted injuryrecommendations made by an ergonomist, while sane or insane; □ insurrection, war or hostilities of any kind, whether declared or not; □ riot or civil commotion, regardless of whether you were participating; □ injury occurring while committing or attempting to commit a criminal offense; □ medical or surgical care which is cosmetic in nature or medical care or surgery that is not medically where necessary. However, periods of disability due Employees must be aware that the committee will require an authorization to the donation of an organ or tissue will be covered. The Company will amend the Group Contract communicate medical information to provide coverage for tubal ligation and vasectomy. (3) Any disability if: □ as a result of injury or sickness for which a third party is liable, except as provided for in the Third Party Liability section; □ you are not under the continuous care and treatment of a physician or specialist who is certified by the Saskatchewan College of Physicians and Surgeons or during the first 30 days under the care of a licensed chiropractor; □ you are imprisoned; □ you are on maternity leave, parental leave or any other leave of absence.
(4) No further benefits will be payable from the date you refuse all members to participate in a rehabilitation program which is considered appropriate by the Insurance Companyadjudication process.

Weekly Indemnity.
