Xxxxxx Insurance. The Employer shall contribute toward the costs for health benefits for each full time employee or SUCCESS employee deemed eligible. Participation in the health benefit is voluntary for each eligible employee or SUCCESS employee. In order to qualify for the Employer’s share of the monthly cost, the employee or SUCCESS employee must qualify under the rules and regulations of the respective carrier or health plan and may enroll in one of the following plans according to the Employer’s current procedures. The employee must complete an application for health insurance within 30 days of date of hire. Provided the application is submitted to the Benefits office of Human Resources within 30 days, coverage will be effective the first of the month following submission of the application. Within fifteen days after hire, t h e district w i l l communicate i n d i v i d u a l l y with all newly hired employees and inform them of the deadline to enroll and their options. Plan 1
