A denial notice shall contain: (i) The specific reason or reasons for the denial of the claim; (ii) Specific reference to pertinent Agreement provisions upon which the denial is based; (iii) A description of any additional material or information necessary to perfect the claim, with an explanation of why the material or information is necessary; and (iv) An explanation of the review procedures provided below.
