Affiliated Physician definition

Affiliated Physician means an individual licensed to practice medicine or osteopathy and who has a contract with the Plan or an IPA to provide services to Members.
Affiliated Physician means any licensed physician that: (i) is performing services on behalf of the Company or its Affiliate pursuant to a written agreement with the Company or such Affiliate, (ii) has an ownership interest in an Affiliate of the Company that performs professional services on its own behalf, or (iii) has a professional practice that is being managed by the Company or an Affiliate, provided, that, in each case, such licensed physician is providing services to the Company, an Affiliate or another entity that is a “service recipient” as defined in Treas. Reg. § 1.409A-1(g) such that common stock of the Company is “service recipient stock” as defined in Treas. Reg. §1.409A-1(b).
Affiliated Physician means a physician employed by or under Contract with an Acquired Company or an Affiliated Physician Practice, as applicable.

  • The Recipient shall keep and maintain all non- financial records and documents relating to the Ministry, the Funding or otherwise to the Service Plan, including without limitation, the health records of each patient of the Recipient, but excluding all patient records in the care and control of an Affiliated Physician, in a confidential manner consistent with all applicable federal, provincial and municipal laws and regulations, and orders, rules and by-laws having the force of law.

  • A written or verbal recommendation by a Medical Group physician or an Affiliated Physician that you obtain non-covered Services (whether Medically Necessary or not) is not considered a referral, and is not covered.

  • This includes Services provided by an Affiliated Physician in the Denver/Boulder or Southern Colorado Service Areas.A referral is limited to a specific Service, treatment, series of treatments and period of time.

  • Maintenance drugs may be obtained for up to the day supply by mail order at the applicable Copayment or Coinsurance, if prescribed by an Affiliated Physician.

Affiliated Physician means any physician participating in a contract with ENH Medical Group, but not as an ENH employed physician.
