Affiliated Physician definition

Affiliated Physician means an individual licensed to practice medicine or osteopathy and who has a contract with the Plan or an IPA to provide services to Members.
Affiliated Physician means a physician employed by or under Contract with an Acquired Company or an Affiliated Physician Practice, as applicable.
Affiliated Physician means any licensed physician that: (i) is performing services on behalf of the Company or its Affiliate pursuant to a written agreement with the Company or such Affiliate, (ii) has an ownership interest in an Affiliate of the Company that performs professional services on its own behalf, or (iii) has a professional practice that is being managed by the Company or an Affiliate, provided, that, in each case, such licensed physician is providing services to the Company, an Affiliate or another entity that is a “service recipient” as defined in Treas. Reg. § 1.409A-1(g) such that common stock of the Company is “service recipient stock” as defined in Treas. Reg. §1.409A-1(b).

  • To the knowledge of the Company, each Affiliated Physician Group and other licensed providers of medical services who currently perform medical services for the Company and each Subsidiary under any Government Contract is duly licensed or certified by the appropriate Government Authority to perform medical services in the applicable state where such person performs such medical services to the extent required by Applicable Law or under any Government Contract.

  • The Executive also agrees that, upon request of any duly authorized officer of the Company, the Executive shall disclose all passwords and passcodes necessary or desirable to enable the Company or any of its Affiliates or the Persons with whom the Company or any of its Affiliates or Affiliated Physician Practices do business to obtain access to the Documents and Third-Party Documents.

  • This warranty of materials, equipment and workmanship is separate from, independent of, and in addition to any other guarantees in this Agreement or any other warranties in this Agreement or required by the Project Documents.

Affiliated Physician means any physician participating in a contract with ENH Medical Group, but not as an ENH employed physician.
Affiliated Physician means a primary care physician legally qualified and licensed to practice medicine or osteopathy and who has a contract with the Plan or an IPA to provide services to Members.
