BLUE CROSS PLUS definition

BLUE CROSS PLUS means a point of service option benefit plan offered by BLUE CROSS under which enrolled Members may, at the time benefits are selected, elect to receive benefits from either a CALIFORNIACARE provider or another licensed provider.

  • Exhibit G, Section II Item E of this Section shall be amended to read as follows: Effective January 1, 2000, total claims for Out-of-Network Expenses rendered to any single BLUE CROSS PLUS Member during the calendar year shall be limited to thirty five thousand dollars ($35,000).

  • Exhibit G, Section II Item F of this Section shall be amended to read as follows: Effective January 1, 2000 the liability of PARTICIPATING MEDICAL GROUP for expenses for Capitation Services rendered during the calendar year to any single Member enrolled in BLUE CROSS PLUS shall be limited to the applicable Enrollment Protection amount defined in Article VIM of the Agreement, as amended.

  • Participating Pharmacy means a pharmacy that has entered into a prescription drug plan agreement with the Pharmacy Benefit Manager listed on your identification card.

  • Medical provider means a medical service provider, a hospital, a medical clinic, or a vendor of medical services.

  • Pharmacy means prescribed drugs and medicines dispensed by a pharmacist and/or travel and allergy vaccines dispensed by a pharmacist or doctor.

  • Participating manufacturer means that term as defined in the master settlement agreement.

  • CCR means the California Code of Regulations.

  • Board of pharmacy or "board" means the Utah State Board of Pharmacy created

  • CMO means a contract manufacturing organization.

  • Network pharmacy means any pharmacy that has an agreement to accept our pharmacy allowance for prescription drugs and diabetic equipment/supplies covered under this agreement. All other pharmacies are NON-NETWORK PHARMACIES. The one exception and for the purpose of specialty Prescription Drugs, only specialty pharmacies that have an agreement to accept our pharmacy allowance are network pharmacies and all others pharmacies are non-network pharmacies.

  • CMS means the Centers for Medicare and Medicaid Services.

  • CMBI means Chase Manhattan Bank International, an indirect wholly-owned subsidiary of Bank, located in Moscow, Russia, and any nominee companies appointed by it.

  • Pharmacy services means the practice of pharmacy as defined in chapter 18.64 RCW and includes any drugs or devices as defined in chapter 18.64 RCW.

  • Clinical peer means a physician or other health care professional who holds a non-restricted license in a state of the United States and in the same or similar specialty as typically manages the medical condition, procedure or treatment under review.

  • SEBI means the Securities and Exchange Board of India.
