Confidential Health Information definition

Confidential Health Information means information pertaining to a patient or client that has been received or created by you or provided by you to another, subject to protection pursuant to “HIPAA”, including an individual’s health, or healthcare treatment information, including the fact that any such individual has been treated by any provider.
Confidential Health Information means all identifiable information relating to a patient participant's health care history, diagnosis, condition, treatment, or evaluation.
Confidential Health Information means information relating to specific Individuals, including Individually Identifiable Health Information, that is exchanged by and between DHS, Trading Partner and/or Business Associate for various business purposes, and that is protected from disclosure to unauthorized persons or entities by Social Security Act § 1171 et seq., the Standards for Privacy of Individually Identifiable Health Information, 45 C.F.R. Confidential Health Information means any patient health information protected by any state or federal law, including, but not limited to patient/insured/member medical records, patient/insured/member claims information,and other information that the Producing Party reasonably believes might containsensitive personal health information. The Parties agree that any Confidential Health Information whether contained in Materials produced in the Action or otherwise shall be deemed “Confidential” and, as such, subject to the terms of this Order. “Confidential Health Information” does not include any Material in which the Producing Party has redacted the identifiers listed above and does not have actual knowledge that the information could be used alone or in combination with other information to identify an individual who is the subject of the information. A Producing Party may, but is not required to, perform such redactions before producing Material that originally contained “Confidential Health Information,” but a Producing Party may not redact information that is relevant to any claim or defense in the Actions.
