Customer Application Form definition

Customer Application Form means the customer application form completed and signed by or for and on your behalf in connection with the provision of the Connexis Connect Service. The Customer Application Form forms part of the Account Opening Documentation.
Customer Application Form means LuxCSD Customer Application Form and LuxCSD Account Application Form;
Customer Application Form means the written instruction of the Customer to AirSpeed to supply the Services in the form of the customer application form attached to this Agreement;
