  • The Settlement Administrator may also allocate and distribute Settlement Funds attributable to a Deceased Class Member’s interests to the estate of the Deceased Class Member, with any such payment to be sent to such mailing address as may be readily ascertainable by the Settlement Administrator.

  • In order to process a claim on behalf of a Deceased Class Member, the Settlement Administrator must receive copies of documents (such as a will, judgment, decree, or other probate documents) that help establish the identity of the Personal Representative of the estate of a Deceased Class Member.

  • You should complete this form if you: (1) are aware that a class member of the class action is deceased (“Deceased Class Member”); and (2) are either a Personal Representative of the estate of the Deceased Class Member, have applied to become a Personal representative of the estate of a Deceased Class Member, or anticipate applying to become or a Personal Representative of the estate of the Deceased Class Member.

  • You must provide the information set forth below and mail the completed form to the Settlement Administrator listed below, postmarked no later than forty (40) days after the initial issuance of the refund check to the Deceased Class Member pursuant to the settlement agreement of this class action.
