Examples of Discharge plan in a sentence
Discharge plan means an individualized plan for post-hospital services that is developed by the CSB in consultation with the individual, guardian or authorized representative if one has been appointed or designated or if one is needed, and the state hospital treatment team.
Discharge plan shall include a summary of goals met/unmet as well as identify what community resources and/or natural supports have been developed to sustain success.
Procedures for the discharge or transfer of the resident shall be established and followed: (II, III)62.14(1) Discharge plan.
Records shall contain at a minimum the following: – The beneficiary’s history; – Evaluation reports; – Clinical documentation (to include treatment plans and reviews); – Service documentation; – Progress notes; – Discharge plan; – Medications; – Documentation of all incidents of restraint and seclusion; and – CON form, and all other required and/or relevant forms.
To be compatible with other regulations for other providers, we are dividing the condition into several standards— Discharge planning evaluation, Discharge plan, Transfer or referral, and Reassessment.
