Eligible student means a student who is eighteen years or older.
Eligible patient means an individual who meets all of the following conditions:
primary education means education offered in years numbered 1 to 7, both inclusive.
Eligible Dependent means a child of an Eligible Retiree who satisfies the requirements for eligibility described in the Eligibility section of this document.
secondary education means attendance at a public or private school offering instruction at grade levels 9-12, or equivalent. (interim eff. 6/6/2010 TL:SR-735; final eff. 7/4/2010 TL:SR-737)
Performance Indicator means a measure of HSP performance for which a Performance Target is set; technical specifications of specific Performance Indicators can be found in the MSAA Indicator Technical Specifications document;
Individualized education program or “IEP” means a written statement for a child with a disability as defined in 14 DE Admin. Code 922.
Participating Orthotic Provider means an Orthotic Provider who has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.
Qualified medical provider means the same as that term is defined in Section 26-61a-102.
