Rebate shall have the same meaning as ascribed thereto in Article 10.3.4 of this Agreement;
Medicare cost report means CMS-2552-10, the cost report for electronic filing of
HMO means health maintenance organization.
Mail order pharmacy means a pharmacy licensed by this
HHSC means the administrative agency established under Chapter 531, Texas Government Code, or its designee.
Seller 401(k) Plan has the meaning set forth in Section 5.5(e).
HSP means HUB subcontracting plan, as described in Chapter 2161 of the Texas Government Code.
Routine patient care costs means Covered Medical Expenses which are typically provided absent a clinical trial and not otherwise excluded under the Policy. Routine patient care costs do not include:
Medicare Levy Surcharge means an extra charge payable by high income earners beyond the standard Medicare Levy if they do not have qualifying private hospital insurance coverage. This charge is assessed as part of an individual or family’s annual tax return.
SSAP means a sporting sanctions appeal panel to be appointed to determine an appeal against a deduction of points under Rule 13.
Capitation means the reimbursement arrangement in which a fixed rate of payment per Enrollee per month is made to the Contractor for the performance of all of the Contractor’s duties and obligations pursuant to this Contract.
Cost-reimbursement contract means a contract under which a contractor is reimbursed for costs which are allowable and allocable in accordance with the contract terms and the provisions of this code, and paid a fee, if any.
Buyer 401(k) Plan has the meaning set forth in Section 6.01(h).
CMS means the Centers for Medicare and Medicaid Services.
MCO means an organization having a certificate of authority or certificate of registration from the 11 Washington State Office of Insurance Commissioner that contracts with HCA under a comprehensive 12 risk contract to provide prepaid health care services to eligible HCA Enrollees under HCA managed 13 care programs.
Premium payment plan means a benefit plan whereby state and

Managed Care Plans means all health maintenance organizations, preferred provider organizations, individual practice associations, competitive medical plans and similar arrangements.
NCAA means the National Collegiate Athletic Association, and its successor. HSA means the Healthy School Act, D.C. Law 18-209 passed by the City Council for the District of Columbia to establish local nutritional standards for school meals;
Managed care plan means a health benefit plan that either requires a covered person to use, or creates incentives, including financial incentives, for a covered person to use health care providers managed, owned, under contract with or employed by the health carrier.
Revenue sharing means sharing tax increment proceeds as defined in ORS 457.470.
Nursing Care Plan means a plan of care developed by a nurse that describes the medical, nursing, psychosocial, and other needs of a child and how those needs shall be met. The Nursing Care Plan includes which tasks shall be taught, assigned, or delegated to the qualified provider or family.
HSD means the New Mexico human services department.
Medicare eligible expenses means expenses of the kinds covered by Medicare Parts A and B, to the extent recognized as reasonable and medically necessary by Medicare.
