Receiving HCBS Waiver Services who Move Outside of the MCOP’s Service Area.
This service will not be duplicative of other services in the HCBS Waiver.
For CP Enrollees who are not in a 1915 (c) Home and Community-Based Services (HCBS) Waiver and who participate in the Home Care Program operated by the Executive Office of Elder Affairs (EOEA), the CP Enrollee’s CP Care Coordinator shall coordinate the provision of LTSS CP supports with the CP Enrollee’s Home Care Program case manager to ensure that LTSS CP supports supplement, but do not duplicate, functions performed by the Home Care Program case manager.
In order to accomplish this, funds shall be provided to persons with developmental disabilities through the State’s HCBS Waiver Program, available federal funds, and/or State funds as necessary.
Pursuant to the schedule set forth below, the State shall provide to individuals with developmental disabilities in the target population the waiver services provided in the State’s HCBS Waiver Program as of the Effective Date of this agreement.
In order to be eligible for Nursing Facility or HCBS Waiver services, an individual must receive at least fifteen (15) points on functional impairment section and a minimum total score of twenty-nine (29) points.
Contractor must abide by the rules and policies provided in each HCBS Waiver.
Items reimbursed with HCBS Waiver funds shall be in addition to any medical equipment and supplies furnished under the State Plan and shall exclude those items, which are not of direct medical or remedial benefit to the individual.
All HCBS Waiver clinical services require a prescription from a Physician.
HCBS Waiver intermittent nursing services are in addition to any Medicaid State Plan nursing services for which the Enrollee may qualify.
