Health Information Custodian definition

Health Information Custodian means a person or organization (described in PHIPA) who has custody or control of Personal Health Information as a result of or in connection with performing the person’s or organization’s powers or duties or the work.
Health Information Custodian. “IPC”, “Minister”, “Personal Health Information” or “PHI”, “Prescribed Entity”, “Research” shall have their respective meanings as under PHIPA or the PHIPA Regulation.
Health Information Custodian or “HIC” means an individual or organization who, as a result of their power or duties, has custody or control of personal health information; healthcare providers such as pharmacists, physicians and nurses are health information custodians; it is the definition used in the Personal Health Information Protection Act, 2004 (Ontario);

  • Health Information Custodian (HIC): As defined in section 3 of the Personal Health Information Protection Act (PHIPA), 2004, a HIC provides CAMH with access to PHI in its custody, and for which CAMH acts as a Service Provider (i.e., an electronic service provider or health information network provider as these terms are defined in PHIPA).

  • Public Health GeneralPersonal Health Information Protection Act (PHIPA)**Any Health Information Custodian (“HIC”) or their agent is authorized to exempt, in whole or in part, any person from the Personal Health Information Protection Act (PHIPA) fees, where he or she is of the opinion that the payment of such fees may cause undue financial hardship to the person requiring the service in question.
