To be eligible for the hospice care benefit, the covered individual must be diagnosed as terminally ill by the attending physician and/or Hospice Medical Director with a medical prognosis of six months or less life expectancy.
For subsequent certification periods, the certification of terminal illness may be made by the Hospice Medical Director or a physician member of the Hospice IDT.
