Hospice Patients definition

Hospice Patients or “Hospice Patient” means an individual whose Attending Physician who is a Doctor of Medicine or osteopathy and Hospice Medical Director certify as having a Terminal Illness and who has elected to receive hospice care.

Examples of Hospice Patients in a sentence

  • The Nursing Home will provide to Hospice Patients, residential and respite nursing home beds as may be needed from time to time by Hospice.

  • Other relations and individuals with significant personal ties to the Hospice Patient may be designated as members of the Hospice Patient's Family by mutual agreement among the Hospice Patient, the relation or individual and the Hospice Team.

  • In the event of termination without cause as detailed above, both parties shall continue to abide by these provisions for any Hospice Patient already receiving services under this Agreement, but no new Hospice Patients shall be admitted under this Agreement after the 30-day notice.

  • Section 2.4 Upon termination, the parties will cooperate with each other to arrange for appropriate follow up care for affected Hospice Patients.

  • This Agreement with its appendices contains the entire agreement between the Parties with respect to the subject matter hereof, and all prior representations or agreements relating hereto have been merged into this document and are thus superseded in totality by this Agreement.

  • Whenever possible, a member of the Hospice IDT will accompany or meet the Hospice Patient(s) at the Hospital/Facility to provide support as needed.

  • Hospice shall ensure adequate orientation and ongoing hospice care training to the LTC Facility's personnel as necessary to facilitate the provision of safe and effective care to Hospice Patients.

  • The Hospital/Facility shall maintain medical records documenting the services that are furnished to Hospice Patients and the family in accordance with this Agreement.

  • The Facility will provide to Hospice Patients, residential and respite 24-hour room and board as may be needed from time to time by Hospice.
