Hospice Team or "Interdisciplinary team" means the following hospice personnel: physician licensed to practice medicine in North Carolina; nurse holding a valid, current license as required by North Carolina law; social worker; clergy member; and trained hospice volunteer. Other health care practitioners may be included on the team as the needs of the patient dictate or at the request of the physician. Other providers of special services may also be included as the needs of the patient dictate.

It must exist primarily to administer a hospice care program meeting the standards of the National Hospice Organization in the patient's home. Related to Hospice Team

  • Hospice means a public agency or private organization licensed pursuant to Chapter 400, Florida Statutes, to provide Hospice services. Such licensed entity must be principally engaged in providing pain relief, symptom management, and supportive services to terminally ill Members and their families.

  • Project Team means Owner, Contractor, A/E and consultants, any separate Contractors employed by Owner, and others employed for the purpose of programming, design, and construction of the Project.   • Project Managers means the person at Developer responsible for the project management of Product and identified in Schedule 2;

  • Project Steering Committee means the committee referred to in Section I.A.1 of Schedule 2 to this Agreement.

  • Team means a team affiliated to a Club, including where a Club provides more than one team in the Competition in accordance with the Rules.
